Undocumented code costs new hires time. In the era of AI agents, it creates a new problem.

Agents do not stop when they lack information. They fill the gaps. They do this with confidence. They do this wrongly.

The reason for a specific architecture stays in people's heads. Your agents cannot see those decisions. They will reverse your logic. They will build on top of your code in ways that break your system.

You must document why your system works this way. Do not write for humans. Write for the agent context window.
